HBsAg阳性人群乙肝病毒前S1抗原检测的临床意义

Clinical Significance of Detection of Pre-S1 Antigen of Hepatitis B Virus in Serum for HBsAg-Positive People

目的 :探讨乙型肝炎病毒前S1抗原与血清乙肝病毒两对半、HBVDNA的相互关系 ,了解乙肝病毒感染者血清Pre-S1Ag的临床意义。方法 :双抗体夹心ELISA法测定血清Pre -S1Ag,EIA法检测血清乙肝病毒两对半 ,荧光定量PCR检测血清乙肝病毒DNA。结果 :112 2例HBsAg阳性人群中 ,Pre -S1Ag阳性率为 5 0 .1%。 114例HBVDNA阳性血清 ,Pre -S1Ag阳性率为 78.1% ,HBVDNA阴性人群 ,Pre -S1Ag阳性率 15 .7% (8/ 5 1) ;HBeAg阳性人群Pre -S1Ag阳性率为82 .2 % ,HBeAg阴性人群Pre-S1Ag阳性率为 35 .7% ;抗 -HBe阳性人群Pre -S1Ag阳性率为 31.6 %。 172例乙肝“两对半”阴性人群Pre -S1Ag阳性率为 1.74 %。结论 :血清Pre -S1Ag与HBVDNA、HBeAg相互关联 ,可以作为HBV存在和复制的标志。联合检测Pre -S1Ag和HBeAg可相互补充、印证 ,对判断乙肝病毒的复制和存在更有价值。血清Pre

Objectve:To discuss the relationships among hepatitis B virus (HBV) Pre-S1Ag, HBV serological markers and HBV-DNA, and the clinical significance of HBV Pre-S1Ag for the people whose HBsAg is positive.Methods:Serum Pre-S1 and HBV serological markers were determined with enzyme immunoassay (EIA) and HBV-DNA was tested with polymerase chain reaction (PCR). Results: The HBV Pre-S1Ag positive rate was 50.1% for 1122 patients whose HBsAg were positive. The HBV Pre-S1Ag positive rate was 78.1% for 114 patients whose HBVDNA were positive. For the people whose HBV-DNA were negative, the HBV Pre-S1Ag positive rate was 15.7%(8/51). The Pre-S1Ag positive rates was 82.2% for the people whose HBeAg was positive and 35.66% for the people whose HBeAg were negative. The Pre-S1Ag positive rate was 31.6% for the people whose anti-HBe was positive.Conclusion:There is a relationship among Pre-S1Ag , HBeAg and HBV-DNA. Pre-S1Ag can be regarded as a mark to estimate whether Hepatitis B Virus is existing or copying, and it can be regarded as a supplementary for HBV-DNA and HBeAg.
