胆囊收缩素在体外循环中的肺保护作用

The protective effect of cholecystokinin pulmonary perfusion during cardiopulmonary bypass on lung injury

摘要 目的 研究体外循环(CPB)术后L-选择素(selectin)的表达以及应用胆囊收缩素(CCK)进行肺灌注,探讨肺损伤发生的机制和CCK的肺保护作用。方法 采用大鼠CPB及术中肺灌注模型,观察术前术后及应用CCK后L-selectin的表达,肺组织超氧化物歧化酶(SOD)活力、丙二醛(MDA)、髓过氧化物酶(MPO)含量变化,氧指数(PaO2/FiO2)、气道压、组织湿干比(W/D ra-tio)及组织学改变。结果CPB术前L-selectin表达为(11.29±4.03)%,术后表达增加为(15.47±4.01)%,P<0.05;应用CCK进行肺灌注后其表达降低为(11.21±3.43)%,与对照组相比差异有显著性(P<0.05),肺损伤相应减轻。结论L-selectin参与了CPB肺损伤的发生,CCK可以减轻CPB肺损伤,改善术后肺功能,它可能干预了L-selectin的表达。 Objective To investigate the mechanism of lung injury and the protective effect of cholecystokinin on lung injury by detecting the expression of L-selectin after cardiopulmonary bypass (CPB) and perfusing lung with cholecystokinin during the procedure. Mathods Model of CPB and pulmonary perfusion in rat was used to observe the expression of L-selectin before and after the procedure, or after cholecystokinin pulmonary infusion, SOD vitality, MDA content, MPO content in lung tissue, PaO2/ FiO2,air way pressure, W/D ratio and histologic changes. Results The expression of L-selectin was increased after CPB, but decreased after cholecystokinin pulmonary infusion during the procedure, and lung injury was attenuated coorespondingly. Conclusion L-selectin may lead to lung injury during CPB, while cholecystokinin may alleviate the lung injury and improve the recovery of lung function by interventing the expression of L-selectin.
