改良扁桃体剥离术治疗慢性扁桃体炎166例

Improved tonsillectomy by dissection for 166 patients with chronic tonsillitis

目的改进扁桃体剥离方法,减轻术后患者的疼痛、出血情况。方法采用扁桃体被膜外棉球填充衬垫剥离术治疗慢性扁桃体炎166例,观察出血量、腭弓腭垂肿胀淤血情况、术创伪膜生长情况、术后始进半流饮食时间。结果观察组术后少量出血104例,中等量62例,无大量出血,腭弓腭垂肿胀轻微,疼痛较轻,术创伪膜12～18h萌出,术后d3始进半流饮食。对照组术后中等量出血123例,较多量37例,大量出血6例,术创伪膜18～24h萌出,术后d5始进半流饮食。结论该方法能同时完成剥离和止血,术后疼痛反应轻、进食早。

Objective To improve the approach of the tonsillectomy by dissection and relieve the patients' pain,and bleeding.Methods The tonsillectomy by dissection with bedding cotton ball in extracapsilar was used in 166 patients with chronic tonsillitis,and the operative bleeding,swelling,stasis of blood,psudomembrane growth and post-operaticve eating time were observed.Results In observed group,there were mild bleeding and swelling of palatal and uvular arch and local painful,and the local psudomembrane may be found on 12～18h after operation ,and the semiliquid diet may be took on third day.In control group, the local psudomembrane may be found on 18～24h,and the the semiliquid diet may be took on fifth day after operation.Conclusion This method can accomplish dieresis and hemostasis simultaneously with less pain, and tanking food earlier after operation.
