摘要
目的 :探讨癫痫临床发作前、中、后的AEEG演变过程及变化规律 ,寻找鉴别癫痫临床发作时痫样波真假的方法 ,提高癫痫阳性的准确率。方法 :总结采用AEEG监测的临床发作患者 75 0例的分析报告体会。结果 :AEEG临床发作捕获率为 2 5 9% ,REEG为 0 11% AEEG癫痫组痫样波发放率 96 6% ,可疑癫痫组2 1 1% ,非癫痫组 5 0 %。本文有 181例 (2 4 13 % )临床发作者为非癫痫性发作。结论 :脑电图在癫痫的诊断和鉴别诊断中是不可缺少的手段 ,比任何其它方法都优越 ,特别是临床发作时的脑电图痫样波发放为癫痫诊断的最基本要素。利用分析临床发作前后脑电活动变化规律的方法 ,不但大大提高了癫痫阳性的准确率 ,而且减少了癫痫诊断的扩大化。
Objective: Researching the AEEG evolution and the AEEG changement law before, in and after epileptic clinic seizure, to find the way that can distinguish epileptic wave true or false in epileptic seizure time and to improve the accuracy of epileptic positiverate. Methods: Sum up analysis report understanding of 750 cases clinic attack patients who have been used AEEG.Results: The capture rate of AEEG clinic attack is 25.9%, when comes to REEG it is 0.11%, AEEG epileptic group's epileptic wave issuness rate is 96.6%, in suspicious epileptic group it is 21.1%, and in unepileptic group it is 5.0%. In this article there are 181 cases (24.13%) are unepileptic attack.Conclusion: Electroencephalography is an indispensable means to diagnose and differential diagnosis epilepsy, which is superior to any other ways. Especially, the electro corticogram epileptic wave in clinic attack time is the most essential element to diagnose epilepsy. Throuqh the way that analyzing the EEG changement law before after or clinic attack, we cannot only greatly improve the accuracy of EEG positive rate but also decrease the expansion of epileptic diagnosis.
