摘要 目的 评价螺旋CT三维重建在急性外伤性颅内血肿中的应用价值。资料与方法 对10 4例急性外伤性颅内血肿患者进行螺旋CT扫描,采用表面遮盖成像(SSD)和表面透视成像Integral技术分别行颅骨、血肿、脑表面的三维重建,伪彩色技术处理后相互合并,在血肿三维空间定位和体积测量后进行手术评估。对需手术治疗者,再术前评估,应用血肿三维空间定位及骨切削技术模拟颅骨开瓣手术,制定手术方案,选择最佳手术入路。结果 全部病例三维重建后满意地显示出血肿立体解剖关系。2 3例手术患者预后满意,术中血肿量与术前测量值基本相同。结论 螺旋CT三维重建简化观察者的思维过程,可直观、立体、准确地显示血肿空间解剖关系和体积值,具有较高的临床应用价值。 Objective To evaluate the clinic value of the three-dimensional reconstruction with helical CT in acute traumatic intracranial hematoma.Materials and Methods Helical CT were performed in 104 patients with acute traumatic intracranial hematoma with SSD and integral technique to reconstruct the three-dimensional image of cranium,hematoma and brain surface respectively, then the three pictures of cranium, hematoma and brain surface were modified by pseudo-color technology and integrated, the space site and size of hematoma were measured. To the patients who needed operation treatment, hematoma were evaluated and the three-dimensional space localization and bone-cut technique were used to imitate the real operation course in order to establish the operation program and optimize the operation route. Results All the three-dimensional reconstruction pictures displayed the stereo-anatomic structure of the hematoma satisfactory. 23 patients who accepted operation treatment had a good prognosis and the size of the hematoma was similar in measurement before and intraoperation. Conclusion The stereo-anatomic structure and size of the hematoma can be displayed satisfactory with helical CT three-dimensional reconstruction, which simplifies the course of viewer's thought and have a high value in clinical application.
