益元保肾方配合福辛普利治疗早期糖尿病肾病的临床研究

Clinical Study of Yiyuanbaoshen Decoction Combined with FosinoprilTreating Early Diabetic Nephropathy

目的:探讨益元保肾方配合福辛普利治疗气阴两虚夹瘀型早期2型糖尿病肾病的临床疗效。方法:选择90例早期2型糖尿病肾病气阴两虚夹瘀证患者,采用随机对照的原则分为治疗组、中药组及西药组,在基本降糖治疗基础上,分别给予益元保肾方配合福辛普利、益元保肾方及福辛普利治疗2月,观察治疗前后早期糖尿病肾病及中医证候疗效,并对尿白蛋白排泄率、血脂及血液流变学等指标进行比较。结果:益元保肾方配合福辛普利治疗早期2型糖尿病肾病总有效率为89.3%,中医证候改善率为96 .4 %,并可降低尿白蛋白排泄率及血脂,改善血液流变学指标,均显著优于中药组及西药组(P <0 .0 5 )。结论:益元保肾方配合福辛普利是治疗气阴两虚夹瘀型早期2型糖尿病肾病的有效方法。

Objective:To discuss the clinical curative effect on early type 2 diabetic nephropathy (DN) with deficiency of Qi and Yin with blood stasis syndrome by using Yiyuanbaoshen decoction combined with fosinopril.Methods:90 cases of inpatients with early type 2 diabetic nephropathy with deficiency of Qi and Yin with blood stasis syndrome from August 2002 to June 2004 were selected and divided into treatment group, Chinese medicine contronl group and western medicine contronl group randomly, and were treated with Yiyuanbaoshen decoction combined with fosinopril, Yiyuanbaoshen decoction and fosinopril respectively for 2 months in additon to the basal reduce-sugar treatment.The curative effect and Chinese medical syndromes were observed. Moreover,urinary-albumin-excretion-rate(UAER),blood lipid and the index level of blood rheology were measured and compared before and after treatment.Results:The total effective rate of Yiyuanbaoshen decoction combined with fosinopril treating early type 2 diabetic nephropathy with deficiency of Qi and Yin with blood stasis Syndrome was 89.3% while Chinese medical syndromes improvement rate was 96.4%. Moreover, it decreased the levels of UAER,blood lipid, and improved the index level of blood rheology, and all of above were more significantly enhanced compared with Chinese medicine control group and western medicine control group(P<0.05).Conclusion:Yiyuanbaoshen decoction combined with fosinopril is effective in treating early type 2 diabetic nephropathy with deficiency of Qi and Yin with blood stasis syndrome.
