利多卡因高钾温血心停搏液对犬冠脉微循环内皮细胞结构和功能的影响

摘要 目的观察利多卡因高钾温血心停搏液对犬冠脉内皮细胞结构和功能的影响。方法12只杂种犬随机分为对照组和利多卡因组。常规建立体外循环(CPB),对照组常规运用ST-ThomasⅡ冷晶体心停搏液,利多卡因组用利多卡因高钾温血心停搏液单次(5ml/kg)灌注心脏,心脏停搏时间均为120min,体外循环辅助30min,停机30min后,结束实验。测定CPB术前、主动脉开放时、停机时、停机后30min血PGI2(前列环素)、ET(内皮素)的含量,脱机后30min取左心尖心肌组织做电镜检查。结果血液中PGI2、ET的含量组间比较无差异(P>0.05),电镜结果显示:两组心肌毛细血管结构完整,内皮细胞无水肿,血管腔通畅。结论利多卡因高钾温血心停搏液对冠脉内皮细胞的结构和功能有良好的保护作用。 Objective To observe effect of lidocaine hyperkalemia warm blood cardioplegia (LHKWBCP) on structure and function of dog coronary endothelial cells. Method 12 dogs were randomly divided into two groups(n=6), control group and lidocaine group. Cardiopulmonary bypass (CPB) was routinely established. Cold crystalloid ST-Thomas II cardioplegia was perfused routinely in control group. Single-dose LHKWBCP was injected in lidocaine group(5ml/kg). Heart was arrested for two hours, CPB assisted circulation for half an hour after releasing aortic crossclamp. Experiments were over 30 minutes after operation. The plasma PGI2?ET levels were measured preoperatively and after releasing aortic crossclamp. Ultrastructure of endothelial cells were observed with electronic-microscope. Results The plasma PGI2 and ET levels weren't statistically significant between two groups. Electronic-microscope showed that the construction of capillary and endothelial cells was intact, capillary wasn't pluged by blood cells. Conclusion LHKWBCP exerted better protective effect on the construction and function of dog coronary endothelial cells.
