腮腺脉管瘤的超声诊断研究 被引量:3

摘要 目的:回顾性分析经手术病理证实的30例腮腺脉管瘤的声像图特征,旨在提高腮腺脉管瘤的诊断符合率。方法:超声观察肿块的形态、质地、内部回声、后方回声、境界包膜和低头试验以及彩色多普勒表现。结果:病变检出率为100%,符合病理学诊断为60%。囊性水瘤有特殊声像图,低头试验阳性13例,此指标对海绵状血管瘤的诊断较敏感,而对毛细血管瘤则相对不敏感。毛细血管瘤和海绵状血管瘤多数能显示内部有丰富的彩色血流信号,呈条状和点状,海绵状血管瘤内部常伴有强回声光团(后方伴声影),常可测得为低速静脉谱;蔓状血管瘤通常可见极丰富的血流信号,可测得有典型或欠典型静脉谱,且流速通常较快,阻力指数较低。结论:诊断的依据主要来自灰阶声像图,多普勒作为参考。超声在腮腺脉管瘤的超声诊断中有很高的应用价值。 Objective: The paper retrospectively analysed 30 cases with parotid vasculolymphatic tumor diagnosed by operateion and pathology. Our purpose was increase ultrasound diagnostic correlation with pathology. Methods: The observation is: mass’s morphogenesis, hardness, margin, capsular, echogeneous, posterior echo and test of head lower heart on ultrasound examine. The blood stream signals on the mass’s internal and margin are showed on CDFI, the typical artery spectrum, typical vein spectrum and non-typical artery spectrum are measured on spectrum doppler. Results: Hgroma has the typical sonography for this structure. Positive rate on test of head lower heart is 13/22 cases. This index is sensitive for the cavernous hemangioma and non-sensitive for the capillary hemangioma. The abundant blood stream signals(dot and stripe) can be showed a lot of the cavernous hemangioma and capillary hemangioma. The high-echo area with posterior shadow and measures vein spectrum of the lower velocity were usually found on the cavernous hemangioma. The abundant blood stream signals and measure typical artery spectrum, semi-typical artery spectrum, the fasting velocity and the lower resistance index were usually found on the cirsoid hemangioma. Conclusions: Utrasound diagnoses mainly come from sonographic feature of parotid’s structure has been changed or not, mass’s all features are observed by us, doppler is reference index. It is valuable for ultrasonic diagnoses of parotid angioma and lymphangioma.
