体表高频超声对兔颈动脉狭窄程度评估的实验研究 被引量:5

摘要 目的:探讨体表高频超声评估兔颈动脉经皮腔内动脉成形术(PTA)后再狭窄(RS)程度的应用价值。方法:47只雄性日本纯种大耳白兔随机分为正常组、模型组和假手术组,应用体表高频超声在动物处死前测定颈动脉内径、膜厚、收缩期峰值流速、舒张末期流速、搏动指数和阻力指数,评估颈动脉狭窄率;术后取颈动脉进行常规组织处理,行HE染色和弹力纤维染色,光镜形态学观察及图像分析仪分析,测定内径、膜厚和狭窄率,对二种方法结果进行统计学分析。结果:超声评估血管轻、中、重度狭窄的敏感性分别为71.43%、81.82%、50%,特异性分别为75.86%、88%、100%,准确性分别为75%、86.11%、87.1%,阳性预测值分别为41.67%、75%、100%,阴性预测值75.86%、91.67%、81.48%。结论:体表高频超声对兔颈动脉PTC后RS狭窄程度的动态鉴别有一定的应用价值。 Objective: To investigate the value of high-frequency ultrasound in elvaluating straitness of rabbits' carotid after PTA. Methods: Forty-seven Japan purbred flap-eared white rabbits were divided randomly into normal control group, model group and fake operation. Inside diameter, membrane thick, peak speed systole, pulse index and resistant index of carotid was measured by ultrasound before killed. Straitness of carotid was evaluated. After operation, the carotid arteries of groups have been researched pathologically. Pathological dyeing: HE and Masson. Then microscope and photo-analyzing research: inside diameter, membrane thick and narrow states. The results evaluated by ultrasound and pathological were compared and analyzed. Results: The sensitivity of the narrow states in light, middle and heavy by ultrasound was 71. 43% ,81. 82% and 50%, the specific was 75.86%,88% and 100%,the validity was 75%, 86.11% and 87.1%, the positive oredict value was 41.67%,75% and 100%, the negative predict value was 75.86%,91.67% and 81.48%. Conclusion: To evaluate the narrow states of rabbits carotid and the effect of experiment drug by high-frequent ultrasound has certainly applied value.
