摘要 目的研究体外循环冠状动脉旁路移植术(on-pumpCABG)围手术期肺通气和弥散功能变化。方法15例体外循环冠状动脉旁路移植术患者,均为左乳内动脉及大隐静脉取材,测量围手术期最大潮气量(FVC)和1min用力潮气量(FEV1)与预测值的比值,以及肺泡—动脉氧分压差[P(A-a)O2]和动脉氧含量(CaO2)指标,并进行分析。结果FVC、FEV1及CaO2指标在术后下降明显,P(A-a)O2术后明显上升,差异有统计学意义。结论体外循环冠状动脉旁路移植术术后肺功能损害严重,应加强围手术期肺脏综合治疗。 Objective To study the changes of the pulmonary ventilation and diffusion function during the periperiod of on-pump CABG. Methods Pulmonary function was tested in 15 patient undergoing on-pump CABG. The FVC, FEV1, P(A-a)O2 and CaO2 were measured and the results were analysed. Results Impairment of pulmonary function was observed with significant decline of FVC, FEV1 and CaO2 and significant elevation of P(A-a)O2. Conclusion There is an evident impairment of pulmonary function in the perioperative course after on-pump CABG surgery, treatment with the lung need to be strengthened during the perioperative period.
