摘要
本文采用放射免疫法测定42例无临床蛋白尿的Ⅱ型糖尿病患者和20例健康人血小板膜表面α-颗粒膜蛋白(GMP-140)分子数和24 h 尿白蛋白定量。结果是①糖尿病组 GMP-140、24h 尿白蛋白排泄(UAE)均明显高于正常对照组(P<0.01);②糖尿病组中,76.2%的患者 GMP-140升高,61.9%的患者 UAE 升高,二者差异显著(P<0.01),GMP-140与 UAE 呈正相关(r=0.91,P<0.01);③在 UAE 升高的糖尿病患者中,96.2%患者 GMP-140升高,而 GMP-140升高的糖尿病患者中,68.8%患者 UAE 升高;④与无视网膜病变组相比,有视网膜病变组 GMP-140、UAE 升高的阳性率增加(P<0.05~0.01)。提示 GMP-140同样可作为Ⅱ型糖尿病肾病早期诊断指标,甚至较 UAE 敏感。
Background To evaluate the association between the level of alpha-granule mem- brane protein(GMP-140),a surface protein of platelet,and urinary albumn excretion (UAE)in patients with diabetic nephropa- thy and to determine if GMP-140 is an indi- cator for early diagnosis of diabetic nephropathy. Experimental design 42 diabetic patients without clinical proteinuria and 20 healthy controls were studied,the levels of GMP- 140 and UAE were determined by radioim- munossay. Results Both GMP-140 and UAE were significantly higher in diabetic patients than in control(P<0.01).GMP-140 was in- creased in 76.2% in diabetic patients while UAE increased in 61.9%(P<0.01). There was a positive correlation between GMP-140 and UAE(r=0.91,P<0.01)in these patients.GMP-140 was increaseed in 96.2% in diabetic patients with UAE in- creased,while UAE was increased in 68.8% in those with GMP-140 increased. The GMP-140 and UAE were higher in pa- tients with diabetic retinopathy than those without retinopathy(P<0.05~0.01). Conclusion The results demonstrate that the GMP-140,like microalbuminuria,is an indicator for early diagnosis of diabetic nephropathy,it may be even nore sensitive than microalbuminuria examination.
