Experimental study on endovenous DIOMED laser treatment

摘要 目的评价静脉腔内激光治疗的安全性和有效性。方法应用810 nm DIOMED激光治疗山羊后肢隐静脉,A组24只,右侧8 W间断脉冲,左侧10 W间断脉冲;B组24只,右侧12 W 间断脉冲,左侧14 W连续脉冲。术后即刻-84 d共8个时段,各取3只。另取3只为正常对照。结果术后各组均无并发症,与正常对照组比较:(1)术后1 d起静脉造影隐静脉全程未显影;(2) 内皮细胞脱落和腔内血栓形成;(3)术后56、84 d,12和14 W组的治疗静脉皱缩,内膜增生,管腔闭锁;(4)12和14 W组在术后3 d起,而8和10 W组仅在术后7 d治疗静脉闭锁率差异有统计学意义 (P<0.05)。结论 DIOMED激光治疗是安全、有效的微创手段。根据治疗静脉的不同,可选择发射功率12 W间断脉冲或14 W连续脉冲。 Objective To evaluate the safety and effectiveness of endovenous laser treatment on goats. Methods 810 nm DIOMED laser treatment was performed on saphenous veins of goat post-limbs. Both groups A and B included 20 goats, with 8-week (W) intermittent pulses on right side and 10-W in termittent pulses on left side in group A, and 12-W intermittent pulses on right side and 14-W continuous pulses on left side in group B. Studies were carried out at 8 post-operative periods, each having 3 goats for experiment. Another saphenous veins in 3 normal goats were taken as controls. Results No complications occurred in each group. Compared with normal controls, ( 1 ) Saphenous vein was not displayed by venog- raphy 1 day post-operation; (2) Endothelial denudation and endovenous thrombosis were confirmed; (3) Veins treated with 12-W intermittent pulses and 14-W continuous pulses showed shrinkage with intimal hyperplasia and obliteration 56 days and 84 days post-operation; (4) Veins treated with 12-W intermittent pulses and 14-W continuous pulses showed significant differences in venous obliterative rate from 3 days to 84 days, and significant difference was only showed in 7 days post-operation in veins treated with 8-W and 10-W intermittent pulses (P 〈 0.05). Conclusion DIOMED laser treatment is a safe and ef fective minimally-invasive method. According to different caliber veins, 12-W intermittent pulses or 14-W continuous pulses could be chosen during treatment.
