对社区高血压患者进行分级管理的做法与体会

The Experience and Practice of Grade-Based Management of Patients with Hypertension in Wenxin District

目的了解社区高血压患者的分级管理情况。方法根据国家高血压防治标准对辖区内高血压患者进行分级,并按分级管理要求进行社区管理,对筛查检出的1372例高血压患者根据危险度分成低危、中危、高危、极高危4组。1年后进行对比。结果经过1年分级管理后,低危、中危、高危、极高危患者与分级管理前差异有显著性意义(P<0·001)。结论分级管理能有效控制血压,减少并发症,提高生命质量。

Objective To understand the grade- based management of patients with hypertension. Methods According to the national standard on prevention and treatment of Hypertension, 1 372 patients with Hypertension were divided into 4 groups, 637 in low risk group （46. 43% ）, 598 in middle risk group （43.59%）, 83 in high risk group （6.05%） and 54 in highest risk group （3.94%）. Results There was a significant difference between pre - and post - the management （ P 〈 0. 001）. Conclusion The grade - based management can effectively control blood pressure, reduce complications, and promote the quality of life.
