嗅神经母细胞瘤组织病理学分级与颈部淋巴结转移的关系

Hymas Grading and Its Impact on Cervical Lymph Node Metastases of Esthesioneuroblastoma

目的 探讨嗅神经母细胞瘸组织病理学分级对颈部淋巴结转移的影响。方法 回顾性地分析我科1993年3月～2004年6月收治的24例嗅神经母细胞瘤病人的病理切片及临床资料，并用Hymas方法进行组织病理学分级，用单项有序RXC表秩和检验，分析组织病理学分级与颈部淋巴结转移的关系。结果 3例病理切片未找到，余21倒病人组织病理学分级：1级1例，2级6例，3级9例，4级5例。5例病人颈部淋巴结转移。组织病理学分级1～2级与3～4级比较，颈部淋巴结转移差异显著（P〈0.05）。结论 嗅神经母细胞瘤组织高度恶性与低度恶性相比，更易出现颈部淋巴结转移。

Objective To discuss the effect of Hymas Grading on cervical lymph rome metastases of esthesioneuroblastoma. Methods A retrospective analysis was given to the pathological sections and clinical data of the 24 patients with esthesioneuroblastoma treated surgically in our department from Mar. 1993 to Jun ,2004;Hymas Grading and PXC test were made to reveal the relationship between Hymas Grading and cervical lymph node metastases, Results The pathological sections of 3 patients out the the 24 were not found;Among the left 21 cases, 1 ease was Hymas Grading Ⅰ ,6 eases were Hymas Grading Ⅱ ,9 were Hymas Grading Ⅲ and 5 were Hymas Grading Ⅳ;There were 5 paitents who had cervical lymph node metastases;the difference between high-grade tumor and the low-grade tumor was not significant （ P 〈 0.05）, Conclusions The high-grade esthesioneuroblastoma is more inclined to have cervical lymph node metastases than the low-grade tumor.
