摘要 目的评估KM-5000D全自动旋转式角膜板层刀制作角膜瓣的可预测性与安全性。方法对接受LASIK手术治疗的近视患者92例(181眼),采用KM-5000D角膜板层刀制作130μm角膜瓣。术中对角膜瓣进行生物学测量,包括角膜瓣厚度、角膜瓣水平径、基底宽度、角膜瞳孔中心与角膜瓣基底的垂直距离以及角膜瓣中心与角膜瞳孔中心距离,并观察相关并发症。结果角膜瓣厚(126.9±5.3)μm,水平直径(9.6±0.6)mm,基底宽(4.7±0.8)mm。角膜瞳孔中央与角膜瓣基底垂直距离(4.35±0.35)mm。没有发生游离瓣、破瓣、纽扣瓣、偏心瓣等严重的角膜瓣相关并发症。所有角膜瓣对合良好。结论KM-5000D全自动旋转式角膜板层刀制作角膜瓣厚度非常接近130μm,大小接近9.6mm,且个体之间的差异很小,对角膜瓣厚度与大小具有良好的预测性,也无严重的相关并发症,KM-5000D角膜板层刀制作角膜瓣是安全的。 objective To evaluate the predictability and safety of corneal flap produced by KM-5000D automatic microkeratome. Methods Ninty-two cases (181 eyes) with myopia were performed LASIK, and the corneal flap with expecting 130 tan thickness were made by KM-5000D automatic microkeratome. The biological characteristics of flap including thickness, diameter, hinge length, the distance between corneal pupil center and hinge, and the distance between the flap center and corneal pupil center were measured during operation. Any complications related with flap were observed. Results The mean thickness of corneal flap were (126.9 ± 5.3)pro, the corneal flap diameter were (9.6 ± 0.6) mm, the hinge length were (4.7± 0.8) mm. The distance between corneal pupil center and hinge were (4.35 ± 0. 35) ram. No severe complications such as free flap, button hole, broken flap, and severe eccentred flap happened. All flap had a good reposition. Conclusion The thickness of corneal flap is closed to 130 tan, and the corneal flap diameter is close to 9.6 mm. The variation between patients is low, so the thickness and diameter of corneal flap made by KM-5000D automatic microkeratome has a good predictability. No seere complications happen, so the corneal flap reshaped by KM-5000D automatic microkeratome is safe.
