摘要 目的:探讨冠状动脉CTA诊断心肌桥的临床价值。材料和方法:使用16排螺旋CT对56例患者行CTA检查,根据CTA上冠状动脉的走行部位,诊断12例心肌桥,其中7例行X线冠状动脉造影,10例使用3D屏气FIESTA序列行冠状动脉MRA检查,比较CTA、MRA和X线冠状动脉造影显示心肌桥的效率。结果:CT显示12例心肌桥共13支壁血管在心肌内走行,其中4例可以被X线冠状动脉造影显示,MRA不能可靠显示心肌桥,仅3例提示心肌桥可能。结论:冠状动脉CTA可以准确显示心肌桥,为临床提供判断心肌缺血的可能原因。 Purpose: To evaluate the application of coronary CT angiography in diagnosing myocardial bridge. Materials and Methods: Twelve patients were diagnosed on coronary CT angiography based on the abnormal course of coronary artery using 16-slice CT scanner, 7 of them underwent conventional x-ray coronary angiography and 10 of them underwent coronary MRA using 3D breath-holding FIESTA sequence. The efficiency of CTA, MRA and x-ray coronary angiogram in diagnosing myocardial bridge was compared. Results: Thirteen tunneled vessels located in myocardium of the 12 patients were detected on CTA, 4 of them could be detected on conventional x-ray angiogram with typical findings. MRI failed to detect the myocardial bridge excepted for 3 suspected vessels. Conclusion: Coronary CT angiography enables an accurate diagnosis of the myocardial bridge and could give a reasonable explanation to the myocardial ischemia.
