中性粒细胞CD_(64)在新生儿败血症诊断中的价值 被引量:7

摘要 目的探讨外周血中性粒细胞CD64表达水平在新生儿败血症早期诊断中的价值。方法败血症患儿36例,根据病史、临床表现、实验室检查确定诊断;另设对照组26例。应用流式细胞仪检测两组外周血中性粒细胞CD64表达水平。结果败血症组外周血中性粒细胞CD64表达为(60.37±22.70)[平均荧光强度(MFI)],显著高于对照组(23.14±5.10)MFI(P<0.001);败血症组恢复期CD64表达水平下降,但仍高于对照组。中性粒细胞CD64对新生儿败血症诊断的敏感性为95.7%,特异性95.8%,阳性预测值95.6%,阴性预测值为95.4%。结论外周血中性粒细胞CD64测定有助于早期诊断新生儿败血症,判断病情、评价疗效和估计预后。 Objective To evaluate the value of peripheral blood neutrophil CD64 level for early diagnosis of neonatal septicemia. Methods Thirty - six cases of neconatal septicemia were recruited into the study according to diagnostic criteria for neonatal septicemia: white blood cell count, C - reactive protein (CRP), platelet count, micro - erythrocyte sedimentatio rate (mESR) and blood culture; 26 cases of hospitalized neonates with non - infectious diseases were enrolled as controls. The level of peripheral blood neutrophil CD64 was measured by flow cytometry, Results The level of petipheral blood neutrophil CD64 in septicemia patients were (60.37±22.70) mean fluorescence intensity (MFI), which were significantly higher than that of control group (23.14±5.10) MFI (P 〈 0.001). The level of CD64 of septicemia significantly decreased between day 10 - 14 of treatment with antibiotics. Diagnostic sensitivity of neutrophil CD64 in neonatal septicemia was 95.7 %, specificity was 95.8 %, positive predictive was 95.6 %, negative predictive was 95.4 %. Conclusion The measurement of peripheral blood neutrophil CD64 may be helpful for early diagnosis, evaluation of severity of infection, observation of therapeutic effects and prediction of prognosis of neonatal septicemia.
