摘要
心内直视手术30例,通过监测脑氧饱和度(rSO_2)、颈内静脉球部血氧饱和度(SjO_2)以及动脉颈内静脉血乳酸差值(AVDL),综合分析心内直视手术期间脑氧供需平衡。结果:(1)在体外循环复温过程中,rSO_2及SjO_2均明显下降;(2)体外循环开始后,动脉血乳酸和颈内静脉血乳酸进行性增加,但在整个手术麻醉过程中AVDL均未见明显增加,提示:在低温体外循环手术麻醉过程中,复温期间有脑氧供需失衡的趋势,应加强监测。术中若维持PaCO_2 4.6~6.0kPa,转流量1.8~2.2L·min^(-1)·m^(-2),MAP3.46~14.63kPa,不会引起AVDL的明显增加。
In order to assess cerebral oxygen balance during open heart surgery, cerebral oxygen saturation (rSO_2), jugular bulb blood oxygen saturation(SjO_2),and lactate differences between arterial blood and jugular bulb blood (AVDL) were monitored in 30 adult patients. The results: (1)rSO_2 and SjO_2 decreased significantly during rewarming period; (2)arterial and jugular bulb blood lactate increased gradually after the onset of CPB,but AVDL did not changed significantly during anesthesia. The results suggeste that there is a trend of cerebral oxygen imbalance during rewarming period,but AVDI. will not increase significantly,if PaCO_2 is kept at 4.6-6.0 kpa,pump flow at 1.8-2.2 L/min·m^2 and MAP at 3.46-14.63 kPa during CPB.
