31例红白血病骨髓涂片的观察──对亚型的探讨

Morphologic features of bone marrow smears──Investigation on subtypes of 31 cases of erythroleukemia

“红白血病”是一种细胞学上表现异质性的疾病群。本文报道３１例骨髓涂片的观察结果，２８例符合红白血病（Ｍ６）的诊断标准，如按非红系细胞再细分，则相当于Ｍ２ａ２１例、Ｍ２ｂ１例、Ｍ４３例、Ｍ５ａ１例、Ｍ５ｂ２例；如计入巨核细胞，则１例符合全髓性白血病；如再考虑细胞形态异常，则６例符合三系增生异常的急性髓性白血病。另外３例不符合Ｍ６的诊断标准，１例为ＤｉＧｕｇｌｉｅｌｍｏ综合征，２例为急性红血病，本文对这些亚型的诊断标准和其他作者提出的亚型分类法进行了讨论。

Erythroleukemia comprises a grorp of diseases with cytologic heterogeneity. We report here the results on the observation of bone marrow smears of 31 cases, among which 28 cases accord with the AML FAB M6.Further stratified classification with non-erythroid blasts indicates 21 cases of M2a, one M2b, three M4,one M5a and two M5b. With megakaryocytes counted, one case coincides with panmyelocytic leukemia.With cytomorphologic abnormalities considered, five cases are consistent with acute myeloid leukemia with trilineage dyspoiesis. Of the other three cases inconsistent with the criteria of M6, one is Di Guglielmo syndrome,the other two, acute erythremic myelosis. The criteria for diagnosis of subtypes and the subclassification protocols proposed by others are discussed.
