摘要
目的探讨16层螺旋CT三维重建(SSD、MPR、VRT)在复杂性骨折中的临床应用价值。方法对22例复杂性骨折经16层螺旋CT检查并将三维重建的影像资料与X线检查及CT轴位(2DCT/SCT)扫描图像进行对比分析。结果22例患者中,SSD、MPR及VRT检查共发现了121处骨折,检出率为100%,X线发现89处骨折,检出率为74.4%。2DCT发现112处,检出率为92.5%。在复杂性骨折特别是跨关节的多骨的粉碎性骨折中,16层螺旋CT三维重建能对骨折、脱位的形态及其与周围的关系进行立体、多角度、多方位的观察,充分显示出骨折的细微结构及移位情况,以及关节脱位的方向。结论16层螺旋CT三维重建对复杂性骨折的显示、诊断及制定治疗方案具有重要的作用和临床意义。
Objective To investigate the clinical significance of three-dimensional reconstruction technique, SSD, MPR and VRT, of 16-slice CT in the diagnosis and treatment of complicated fractures. Methods A retrospective analysis was done for 22 cases of complicated fractures who received both the X-ray examination and the 16-slice spiral CT examination. Their plain films, 2D transaxial reconstruction CT film and 3D reconstruction CT film both based on 16 slice CT examination were compared and analyzed. Results In 22 cases, 3D reconstruction (SSD, MPR, and VRT) showed 121 fractures, X-ray plain film 89 fractures , and 2D CT film 112 fractures, with their detecting rate 100%,74.4% and 92.5% respectively.
Moreover, the more meaningful thing is that 3D reconstruction technique can well displayed complicated fracture, particular cross-joint fracture, the position relationship between bone pieces, and joint and the around soft tissue from any angle and position. Conclusions The 3D reconstruction technique of 16-slice spiral CT scan is of great importance and clinical significance to the appearance, diagnosis and establish-ment of surgical treatment.
