摘要
目的探讨化疗对肺癌患者外周血Th1/Th2的影响和茶多酚的调节作用。方法应用流式细胞技术分别检测45例肺癌患者和30名正常人(对照组)外周血Th1、Th2细胞因子以及化疗后肺癌患者外周血中Th1/Th2细胞因子的表达,观察茶多酚的调节作用。结果45例肺癌患者外周血中CD4+T淋巴细胞、Th1、Th2细胞的水平分别为1.0±0.31、7.3±2.1和5.1±2.2,对照组外周血中CD4+T淋巴细胞、Th1、Th2细胞的水平分别为1.9±0.11、14.9±5.5和3.3±1.6(P<0.01)。随着肿瘤的不断发展,Ⅲ~Ⅳ期Th1细胞的水平明显低于Ⅰ~Ⅱ期,Th2细胞的水平高于Ⅰ~Ⅱ期。肺癌伴淋巴结转移者其Th2细胞的水平高于不伴淋巴结转移者(P<0.01),而Th1细胞的水平却明显低于不伴淋巴结转移者(P<0.01)。化疗后肺癌患者外周血中CD4+T淋巴细胞的水平(0.42±0.12和0.67±0.23)明显低于化疗前的水平(1.0±0.29和1.0±0.32),P<0.01;化疗后肺癌患者外周血中Th1、Th2细胞的水平与化疗前比较,差异有显著性(P<0.01或<0.05)。结论肺癌患者体内存在着Th1/Th2的失衡,化疗可影响机体的免疫功能,茶多酚具有上调作用。
Objective To explore the effect of chemotherapy on Th1/Th2 cytokine expression in peripheral blood of patients with lung cancer and the regulative study of tea polyphenols(TP) on it. Methotis The expression of Th1 and Th2 in peripheral blood lymphocyte cells from 45 patients with lung cancer and 30 normal donors were detected with immunologically double fluorescent labeled by flow cytometry. The Th1/Th2 cytokine cells in peripheral blood lymphocyte cells from patients with post-chemotherapy after cultured with TP were also detected. Results The levels of CD4^+ T lymphocyte cells ,Th1 cells and Th2 cells in peripheral blood lymphocyte cells from patients with lung cancer were 1.0 ± 0.31,7.3 ± 2.1 and 5.1±2.2,respectively,the levels of CD4+ T lymphocyte cells、Th1 cells and Th2 cells in peripheral blood from normal control group were 1.9±0. 11,14.9±5.5 and 3.3±1.6,respectively. Compared with normal control group, there was a distinctly statistic difference ( P 〈 0.01 ). With tumor progression gradually, Distinctly statistic difference was showed between Th1/Tb.2 cells in stage Ⅲ,Ⅳ and in stage Ⅰ,Ⅱ ( P 〈 0. 01 ). Lung cancer with lymph node metastasis showed lower levels of Th1 cells than lung carcinoma without lymph node metastasis,Th2 cells higher than without lymph node metastasis. The levels of CD4^+ T lymphocyte cells in peripheral blood of patients of post-chemotherapy ( their values were 0.42 ± 0. 12 and 0.67 ±0.23) lower than those in pro-chemotherapy ( their values were 1.0 ± 0. 29 and 1.0 ± 0.32 ), whereas there was distinctly statistic difference between Th1/Th2 cells in post-chemotherapy and in pro-chemotherapy(P 〈 0.01 or 〈 0.05 ). Conclusion The lung cancer patients had a malfunction in Th1/Th2 cells, chemotherapy might impaired the immune function, and TP could regulate the imbalance state of Th1/Th2 cytokine expression in lung cancer patients.
