测定血清胱抑素C的浓度评价肾小球滤过功能 被引量:11

Detecting Serous Levels of Cystatin C and Exploring its Role in the Evaluation of Glomerular Filtration Function

摘要 目的探讨血清胱抑素C(cystatinC,CysC)的浓度在判断肾小球滤过功能中的价值。方法对117例患者分别用乳胶颗粒散射免疫比浊法测定血清CysC的浓度及用酶法测定血清肌酐(Scr)、尿素氮(BUN),根据C0ckCroft-Gault公式计算内生肌酐清除率(Ccr);依据Ccr分组比较。结果随着Ccr下降血清CysC、BUN、Scr均值逐渐升高(P<0.01),血清CysC与Ccr之间呈负相关(r=-0.486,P<0.01),血清CysC与Scr之间呈正相关(r=0.857,P<0.01)。A组25例患者中血清CysC的浓度异常者占19.3%,Scr的浓度异常者0;B组56例患者中血清CysC的浓度异常者占56.3?,Scr的浓度异常者11%;C组28例患者血清CysC的浓度异常者占70%,Scr的浓度异常者50%;D组二者的浓度异常者均为100%。结论测定血清胱抑素C的浓度用来评价肾小球滤过功能是一种可行、可靠、敏感的指标,测定方法简便,对预测早期肾损害迅速准确,值得临床推广应用。 Objective To investigate clinical significance of serous level of cystatin C (CysC) in the evaluation of glomerular filtration function. Methods The serous levels of CysC, creatinine (SCr) and blood urea nitrogen (BUN) were detected by latex particle scattering immune assay and enzymic methods on 117 patients, respectively. By using C0ckCroft-Gault formula, the creatinine clearance (Ccr) was calculated. These patients were divided into four groups based on their levels of Ccr. Results The serous mean levels of CysC, BUN and Scr increased gradually when Ccr decreased ( P 〈 0.01 ). The serous levels of CysC were negative correlation with Ccr( r = - 0.486, P 〈 0.01 ), but positive with Scr levels ( r = 0. 857,P 〈 0.01 ). The ratios of abnormal levels of CysC and Scr were 19.3 % and 0,56.3 % and 11% , 70% and 50%, 100% and 100% ,in group A( n = 25), B ( n = 56) , C ( n = 28) and D ( n = 8), respectively. Conclusions Detecting serous level of Cys C is a convenient, feasible, reliable and sensitive method, which could be applied to evaluate glomerular filtration function. The detection could evaluate the inchoate damages on renal function quickly and exactly. It is worth of clinical spreading and application.
