多药耐药基因在大肠癌组织中的表达及其体视学测定研究

摘要 目的:探讨两种多药耐药基因在大肠癌组织中的表达及对其肿瘤细胞进行体视学分析。方法:采用微波EnVision免疫组化法联合检测63例大肠癌患者癌组织中P-糖蛋白(P-gp)、多药耐药相关蛋白(MRP)的表达水平。运用图像分析系统在显微镜下测量肿瘤细胞的体视学参数,依据有关体视学公式计算出参数核的体密度(Vvn)、核的表面积密度(Svn)、核的平均体积(V)、核的平均表面积(S)、核的平均自由程(")、核的数密度(Nv)、核的平均曲度密度(Kv),比较这些参数在不同分化肿瘤中的差异。结果:P-gp和MRP在大肠癌组织中的阳性表达率分别为54%和30%。参数Vvn、Svn、V、S依高、中、低分化的顺序增加,Kv,Nv、λ依高、中、低分化的顺序减小,Vvn、Svn参数的比较差异有显著性(P<0.05)。结论:大肠癌组织表达多种耐药因子,各耐药因子间共表达具有明显相关性。所有的多药耐药因子表达均与大肠癌分化程度有密切关系,肿瘤细胞Vvn、Svn参数是肿瘤细胞分化的重要指标,且为临床形态学诊断提供了依据。 Objective To investigate the expressions of MDR gene in colorectal carcinoma and to stereologically analyze the related malignant cells. Methods Immunohistochemical staining (EnVision method) was used to detect the expressions of P-gly-coprotein (P-gp) and muhidrug resistance-associated protein (MRP) in cancerous tissues from 63 patients with colorectal carcinoma. The tumorous cells were stereologically measured with the image analyzing system and microscope. The parameters including Vvn, Svn,λ, Nv, Kv, V, and S were calculated based on the formula of stereology and the differences in these parameters were compared among various differentiated cells. Results The positive rates of Pgp and MRP were 54% and 30%, respectively. Vvn, Svn, V, S raised and Kv, Nv, λ decreased aceording to the decrease of differentiation of cells (P 〈 0.05). Conclusions The co-expressions of P-gp and MRP in colorectal cancerous tissues have a marked correlation. Their expressions are all closely related to differentiation of the cancerous cells. Either Vvn or Svn is an important indicator for cell differentiation and provides clinical support for morphological diagnosis.
