三维适形放疗联合GP方案同步治疗局部晚期非小细胞肺癌临床观察

Clinical observation of three dimensional conformal radiotherapy combined with GP regimen chemotherapy in treatment of locally advanced non-small cell lung cancer.

目的探讨三维适形放疗联合GP方案同步化疗治疗局部晚期非小细胞肺癌的近期疗效和急性毒副反应。方法40例Ⅲ期非小细胞肺癌患者采用三维适形放疗，同时予GP方案化疗（G：吉西他滨1000mg／m^2，第l，8天；P：顺铂30mg／m^2，第1-3天，每28天1个周期）。结果肺原发灶总有效率为85％；纵隔转移淋巴结总有效率为91．9％，急性毒副反应主要是骨髓抑制、胃肠道反应和轻中度放射性食管炎、放射性肺炎，均可耐受。结论三维适形放疗与GP方案同步放化疗治疗局部晚期非小细胞肺癌有较好近期疗效。

To explore clinical short-term effect and acute side effects of three dimensional conformal radiotherapy （3DCRT） （ combined with GP regimen chemotherapy in treatment of locally advanced non-small cell lung cancer （NSCLC）. Methods Forty patients of NSCLC Stage Ⅲ were treated with 3DCRT combined with GP regimen chemotherapy （ G：gcmeitabine 1000 mg,/m2 d1：8 ; P：eisplatin 30mg/m2 dl ~3. Each cycle took 20 days in completion. Results The overall response rate was 85% for primary tumor, and 91.9% for metastatic mediastinal lymph nodes. The acute side effects were mostly bone marrow suppression, nausea, vomiting, radiation-induced esophagitis and pneumonitis （ RTOPG I / Ⅲ ） with good tolerance. Conclusion 3DCRT combined with GP is well tolerated in most patients with locally advanced NSCLC. Its short-term clinical effect is good.
