摘要
目的探讨B超测量多项参数,推导预测胎儿孕周公式,以评价胎儿宫内发育状况。方法B超测量98例正常妊娠单胎双顶径、头围、胸径、胸围、腹径、腹围、股骨长、胎盘厚度、羊水量等妊娠生物参数,采用多因素分析法,建立孕周的多元回归方程公式^y=-2.2579+3.0781X2+1.6499X4-0.6017X10。应用公式测算31例胎儿孕周,以验证其符合率。结果对孕周影响大的前三位因素依次是双顶径、羊水量、胸径,而股骨长则居所有因素之末位。标准差(Sy)=3.0373(周)(RR=0.9208,F=174.52,P〈0.01)若测得的孕周(y^)低于实际孕周3周以上,为胎儿宫内发育落后,临床验证符合率为70.97%。结论应用B超多项参数评价胎儿宫内发育状况有综合代表性且有定量的作用,可作为既全面又简便易行的产前监测法。
Objective To establish gestational week calculation equation for the purpose of evaluation of fetus growth status by Type - B Ultrasound data. Methods Multiple - factor regression equation was formed based on multiple - factor analysis of Type - B ultrasound data from 98 nornal preganencies, biological indicators including biparietal diameter, head circumference, thoracic diameter, thoracis circumference, abdominal diameter, abdomincal circumference, femur length, placenta thickness and amniotic fluid quantity were determined. Fol- lowing preliminary theoretic discussion, gestational week calculation equation was established through stepwise regression, i. e. y = -2. 2579 + 3.0781X2 + 1. 6499X4 - 0. 6017X10. The equation was then applied in calculation of gestational week of 31 pregnancies in order to check the coincident rate. Results The three most important impact factors in the gestational week calculation are biparietal diameter, amniotic fluid quantity and thoracic circumference, while the least impact factor is femur length. If calculated gestational week is less than the actual gestational week 3 weeks, it indicates fetal growth delay. Clinical verification shows that the coincident rate is 70.97%. Conclusions Type-B Uhrasound data can be used for quantitative evaluation of fetus growth status and this is a general and easy method to be used in pre - labor monitoring.
