摘要
目的探讨膀胱移行细胞癌(BTCC)组织中Livin的表达及其在BTCC发展及预后中的作用。方法应用免疫荧光法、蛋白印迹法及逆转录聚合酶链反应法,对48例临床分期为Tis~T1期的膀胱癌组织标本中Livin的表达进行检测。病理分级G1级23例,G2级17例,G3级8例;复发性膀胱癌17例;15例非肿瘤膀胱组织作对照。结果15例非肿瘤膀胱组织Livin表达均为阴性;48例膀胱癌组织中有19例可见Livin表达;复发的膀胱癌组织中Livin阳性表达率(58.82%,10/17)明显高于初发的膀胱癌组织(29.03%,9/31),差异有统计学意义(P<0.05);Livin表达率在不同分级的肿瘤组织中无明显差异。2年随访发现:Livin表达阳性的肿瘤病人复发率高(13/19,68.42%),与Livin表达阴性者的复发率(11/29,37.93%)相比差异有统计学意义(P<0.05)。结论Livin与膀胱癌复发有关,检测膀胱癌组织中Livin表达有望作为膀胱癌诊断的一种指标,在膀胱肿瘤的预后判断中起重要作用。
Objective To evaluate the expression of Livin in bladder transitional cell carcinoma (BTCC) and to investigate its clinical and prognostic implications. Methods: The expression of Livin was detected in 48 cases (33 men and 15 women)of BTCC and 15 cases of non tumor bladder tissues(controls) by immunofluorescenee, Western blotting and RT-PCR method. The average age of the 48 cases at diagnosis was 59 years,with the clinical stage of Tis-T1 in all 48 cases and the pathological grades of G1 in 23 cases,G2 in 17 cases and G3 in 8 cases. Of the 48 cases, 17 developed relapse. Results No Livin expression was detected in 15 non tumor bladder tissues. In 48 cases of BTCC,the expression of Livin was detected in 19 cases: 39.13% (9/23) in G1 cases,41.18%(7/17) in G2 cases and 37.50%(3/8) in G3 cases, respectively ,without significant differenee(P 〉 0.05 ). The expression of Livin in relapse cases was 58.82% (10/17), in primary cases was 29.03% (9/31). There was a statistically significant difference between them (P 〈 0.05). It was found in 2 years of follow-up that the rate of relapse was 68.42% (13/19) in positive Livin expression cases,and 37.930/o (11/29) in negative Livin expression cases,with a statistically significant difference between them (P 〈 0.05 ). Conclusion The expression of Livin is associated with the relapse of BTCC, and Livin may serve as a marker for diagnosis of BTCC and play an essential role in the prognosis of BTCC relapses.
