摘要 目的探讨16层螺旋CT在诊断上颌窦疾病中的临床应用价值。方法回顾79例手术或临床随访证实上颌窦疾病患者用飞利浦brilliance16层螺旋CT按颌面部常规条件扫描后进行多平面重组成像、容积成像技术、CT仿真内窥境等图像后处理,并由2位以上CT诊断医生进行比较和综合分析。结果79例患者中,慢性炎性病变59例,良性病变4例,上颌窦癌2例,上颌窦外伤14例。16层螺旋CT定位诊断正确率为100%,定性诊断正确率89.87%。冠状位重组图像是观察上颌窦解剖结构、软组织、骨组织、病变的显示情况最佳成像方法。结论16层螺旋CT重建能提供上颌窦的空间关系信息,可作出定性定量诊断,临床应用价值大,有利于临床医师选择治疗方案。 Objective To investigate the applied value of 16-slice spiral CT in the diagnosis of diseases about maxillary sinus. Methods 79 cases with maxillary sinus diseases confirmed by surgery or follow-up, All cases were performed in volume scan according to conventional maxillofacial scan by phillps brilliance 16-slice CT and three-dimensional(3D) imaging including multi-planar reconstruction, volume rendering technique, CT virtual endoscopy, all the pictures were observed and analyzed by two radiologists. Results Among 79 cases, there were 59 cases with inflammatory diseases; 4 case with benign tumors; 2 cases with malignant tumors and 14 cases with fractures. The diagnosis accordance rate for location was 100%, and for the nature of the disease was 89. 87%. Coronal images reorganization of the maxillary sinus is the best imaging method to observing ana- tomical structure, soft tissue, bone tissue and the disease situation. Conclusion 16-slice CT could provide valuable spatial information of the maxillary sinus, to make a qualitative and quantitative diagnosis, clinical value, and help clinicians choosing treatment.
