老年肿瘤术前应用苯巴比妥钠与咪达唑仑的对比性研究

The comparison of using Phenobarbital and Midazolam before taking operation in old age tumorous patients

目的观察老年肿瘤患者术前30分钟使用阿托品、苯巴妥与阿托品、咪达唑仑对呼吸循环的影响。方法120例肿瘤手术患者,年龄60～81岁,随机分为对照组(肌肉注射苯巴比妥100mg与阿托品0.5mg)及实验级(肌肉注射咪达唑仑0.05mg/kg与阿托品0.5mg),两组患者分别于注射后10min、20min、30min用SIEMENS7250多功能监护仪(德国)观察无创血压、平均动脉压、心率、脉搏、血氧饱和度,镇静评分及遗忘程度。结果镇静评分结果显示:实验组有统计学意义(P=0.000),对照组无统计学意义;遗忘程度结果显示:实验组没有统计学意义P=0.289),达到完全遗忘;对照组有统计学意义(P=0.005),说明遗忘程度有差异。用药前后试验组心率、血压均有不同程度的降低,有统计学意义。使用药物前后各组血氧浓度没有变化,没有统计学意义。结论本研究显示咪达唑仑复合阿托品作为术前用药能够达到效果可靠、且可产生100%遗忘率,其镇静和抗焦虑效果好,特别适合老年患者术前应用。

Objects To observe the effect on respiration and circulation of old age tumorous patients before 30 min of taking operation.Methods 120 cases tumorous patients who would take operation,age from 65 to 60 were separated into control group（IM Phenobarbital 100mg and Atropine 0.5mg before 30 min of taking operation） and experimental group（IM Midazolam 0.05mg/kg and Atropine 0.5mg before 30 rain of taking oeration.） at random.Two groups were observe blood pressure,mean arterial blood pressure,heart rate,pulse,saturation of blood oxygen,the score of sedation and the degree of lapsus memoriae after injectining respectively 10min,20min,30min by multipurpose monitor （SIEMENS7250,Germany）.Results Results of sedative score shows there had a statistical significance in experimental group （P=0.000）and no significance in control group.Results of lapsus memoriae degree shows there had no statistical significance experimental group （P=0.289） and had a statistical significance in control group （P=0.005）,which indicates that inexperimental group had been total amnesia.There had a different low before ijjection and agter injection 10min,20min,20min in heart rate and blood pressure which indicates having a statistical significance;and there had no change in saturation of blood oxygen before injection and after injection Conclusins There had a indication that Midazolam compounding Atropine as Preoperative Medication had got reliable effect,good sedation and a ntianxiety,100% lapsus meomoriae rate,which specially fit in with preoperative of old age trmorous patients.
