珠蛋白合成障碍贫血患儿去白红细胞输注与FNHTR的相关性研究 被引量:7

摘要 目的评估去除白细胞输血的临床应用价值。方法取充分混匀的过滤前、后血袋内血样各3ml,分别用血球计数仪计数白细胞、红细胞和血红蛋白,分析经白细胞过滤器过滤前后供者相关血液参数的变化。同时,将78例(398人次)输注未滤除白细胞血液的地中海贫血患儿作为对照组;127例(645人次)输注滤除白细胞血液的患儿作为观察组,观察两组患儿的输血反应情况。结果过滤后白细胞去除率平均为97.9%,红细胞回收率为92.8%,血红蛋白过滤后的差异无统计学意义(P>0.05)。对照组与观察组的FNHTR发生率分别为21.8%和0.8%,差异有统计学意义(P<0.01)。结论白细胞滤器能有效去除白细胞,预防输血反应发生。 Objective To evaluate the application of leukocyte removal filter in elinieal blood transfusion. Methods The 3 ml blood samples were taken before and after filtering respectively. Relative parameters,such as leukocytes,red blood cells and hemoglobin were counted by a blood counter. Relative parameters of donor's blood were compared before and after leukocyte removal filter. Meanwhile, 398 eases of thalassemia children were transfused with leukoeytes non-removed blood as control group, 645 thalassemia children were transfused with leukocytes removed blood as observational group. The recipients were monitored for FNHTR for two days. Results The removal rate of leukocyte was 97.9% after filter. The recovery rate of red blood cell was 99. 0% ,but there was no significant change in hematoglobin level(P〉0. 05). FNHTR rate of control group was 21.8% and that of observational group was 0. 8%. There was significant difference between the two groups(P〈0. 01). Conclusion Leukoeyte removal filter can effectively remove leukoeytes from blood and prevent the FNHTR.
