全麻复合硬膜外阻滞麻醉诱导期血液动力学的变化 被引量:2

摘要 目的观察全身麻醉、复合硬膜外阻滞麻醉(CGEA)气管插管时的心血管反应,以及两种麻醉并行初期血压的改变。方法选ASAⅠ~Ⅱ级开胸、上腹部手术50例。随机分为两组,全麻组25例,为单纯全身麻醉;全硬组25例,先行硬膜外麻醉,然后再行全身麻醉。测定插管前后平均动脉压(MAP)、心率(HR)的改变。结果全硬组MAP在插管前、插管后1、5分钟显著低于全麻组(P<0.05);插管后5、10分钟HR显著慢于全麻组(P<0.05);全麻组插管后10分钟仍未恢复到插管前水平(P<0.05)。全硬组有15例出现低血压,需用麻黄素纠正。结论CGEA可减轻气管插管引起的心血管应激反应。 bjectives:The obseerations are made on hemodynamics reaction when making general compound epidicral anesthesia (CGEA) ttached intubation and blood pressure(BP) change during the begining of both anesthesias occurence. Methods Selecting 50 cases of Anesthetist society of American(ASA)Ⅰ~Ⅱchest operations and upper abdomen operatrions divinding two groups in Random General anesthesia, n=25, only for general anes thesia;CGEA,n=25,doing epidural anesthesia firstly and then general anesthesia.Determing the changes of medium arterial pressure(MAP)and heart rate (HR)before and after trached intubation. ResultsIn the latter group MAP is clearly lower than that of the first group before and afterl-5 minutes trached intubation(P<0.05),and HR is slower than that of the first group,after 5-10 minutes' trached intubation(P<0.05);thet first group HR doesn't recover its orignal levelP<0.05) after 10 minutes' trached Intubation.15cases of low BP occur Inthe latter group,and also need dphedrine to adjust as well.Conclutions CGEA will ease the alarm reaction of hemodynamics caused by trached intubation,and be likely to cause in low BP.
