摘要
目的比较不同疼痛强度对晚期胃肠道癌症患者生活质量的影响。方法根据WHO的标准将疼痛程度分为4级,采用生活质量核心量表对200名住院晚期胃肠道癌症患者进行生活质量调查。结果本组无痛56例,轻度疼痛101例,中度疼痛34例,重度疼痛9例。随着疼痛强度的增加,患者总体生活质量的得分由(46.354±13.194)逐渐下降为(17.593±14.699)。不同疼痛强度之间患者总体生活质量的方差分析显示,除无痛与轻度疼痛之间患者总体生活质量差异无统计学意义(P〉0.05),其余疼痛强度对患者总体生活质量影响的两两比较差异有统计学意义(P〈0.01)。结论不同疼痛强度对晚期胃肠道癌症患者的总体生活质量有不同的影响,疼痛强度与总体生活质量之间呈反比;医护人员应重视疼痛对患者生活质量的影响,提高患者的生活质量。
Objective To study the influence of different degrees of pain on the quality of life (QOL) in patients with advanced gastrointestinal cancer. Methods The degrees of pain were grouped into four levels according to the WHO standard. The QOL of 200 advanced gastrointestinal cancer patients in hospital was assessed by EORTC QLQ-C,30. Results There were 56 eases with no pain, 101 cases with mild pain, 34 cases with mederate pain and 9 cases with severe pain. The QOL of advanced gastrointestinal cancer patients is poorer. With the increasing degrees of pain, the total score of QOL in patients with advanced gastrointestinal cancer decreased graduately from(46.354 ± 13. 194)to ( 17. 593 ± 14.699). The analysis of variance on total quality of life among different degrees of pain showed that there was no significant difference between no pain and mild pain in patients with advanced gastrointestinal cancer (P 〉 0. 05 ). There were significant differences between no pain and moderate pain (P 〈 0. 05), between no pain and severe pain (P 〈 0. 05 ), between mild pain and moderate pain (P 〈 0. 05 ), between mild pain and moderate pain (P 〈 0. 05 ), between moderate pain and severe pain ( P 〈 0. 01 ). Conclusions Different degrees of pain had different influences on the total quality of life in patients With advanced gastrointestinal cancer. The total score of QOL is negatively correlated with different degrees of pain. It should pay more attention to care and support the advanced gastrointestinal cancer patients and seek active measures to improve their QOL.
