摘要
目的探讨腹腔感染病人合并低白蛋白血症的机理。方法以大鼠盲肠结扎穿孔(CLP)为模型,观察CLP后白蛋白mRNA表达的变化,并应用回归和相关方法,分析激素、细胞因子和内毒素的作用。结果 CLP后白蛋白mRNA表达显著下降,并与血浆白蛋白浓度的变化密切相关,白蛋白mRNA的变化与门脉血内毒素明显相关,而与血中胰岛素、胰高糖素、胰高糖素/胰岛素比值、皮质醇、TNF、IL-1和Ⅱ-6浓度的变化无明显相关。结论白蛋白mRNA表达受抑是低白蛋白血症的重要原因,而内毒素是重要的影响因素。
Objective To investigate the influence of peritoneal spesis on albumin gene expression and its mechanism. Methods, The model of cecal ligation and punture (CLP) of rats was used to observe the changes in albumin m RNA expression after CLP. The roles of hormones, cytokines and endotoxin were studied by correlation and multiple regression. Results The albumin mRNA expression was markedly inhibited after CI.P, which was closely correlated with the changes of serum albumin concentration. The decreased albumin mRNA was closely related with the increase in portal endotoxin concentration, and not with the changes in serum insulin, glucagon, cortisol, TNF, IL-1 and IL-6 concentrations after CLP. Conclusion The inhibition of albumin mRNA expression is a key cause of hypoalbuminemia in peri- toneal sepsis which probably result from the increase of portal endotoxin concentration.
