糖尿病足61例临床分析

Clinical characteristics of 61 diabetic foot patients

目的调查和分析我院糖尿病足患者的临床特点。方法调查我院1999年9月至2007年12月糖尿病足患者的特点，包括病史回顾、体格检查、生化检查及医疗费用等，对足溃疡进行分类、分期和病因分析。结果共调查糖尿病足患者61例。糖尿病足患者〉50岁占80％，以男性为主，糖尿病病程较长，文化程度偏低，常见诱因以物理因素多见，分泌物进行细菌培养阳性率高，住院天数长及费用高，平均住院费用7085元。结论糖尿病足患者多为高龄、男性、文化程度低、血糖控制差者，足溃疡合并感染率高、医疗花费大，以药品花费最多。

Objective To investigate the clinical characteristics of diabetic foot in our hospital. Methods Diabetic patients with foot problems and/or peripher alartery diseases were surveyed from September, 1999 to December, 2007 in our hospital,including demographic data, present and past history of the foot and peripher alartery diseases, the classification and phases of the foot ulcers based on the Wagner system, control of the hyperglycemia and lipid disorder, medical cost in hospital and the diabetic complications and relativerisk factors. Results In the study, 61 diabetic patients with foot problems were analyzed. In diabetic foot group, male had more diabetic food than female, foot problems most frequently occurred in the elderly diabetic patients with long duration, lower educational level. Ulcers were complicated with infection. The average direct medical cost in hospital for the diabetic patients with foot problems was RMB 7085 yuan. Conclusions Foot problems markedly increase the medical cost of the diabetic patients. The elderly diabetic men＇ s patients with long dura- tion, lower educational level, restrict control of diet and should be paid more attention on intensifying foot care and preventing education of foot problems.
