摘要
目的:回顾本课题组于1996~2008年完成的传统针灸助产处方(合谷-三阴交)优化研究,展望今后的研究方向。方法:收集本课题组发表的相关论文21篇,从文献、临床、基础3个方面的研究进行全面总结与分析。结果:文献研究:在临床使用的下胎穴中,从古至今合谷、三阴交穴均位列第一;临床研究:电针合谷.三阴交穴可促进子宫协调收缩、缩短产程、增强药流;基础研究:上述作用的发挥可能与有效地抑制子宫无序收缩、增强子宫有效收缩、影响机体内前列腺素E2、雌二醇等激素水平有关;总结出不同针刺顺序、下针先后的间隔时间、不同留针时间、不同电针连线方式、不同电针刺激参数均对疗效产生影响;经过临床.基础-临床的反复研究、探讨和验证,获得该处方的初步优化方案:先合谷穴20min,后加三阴交穴5min,电针采用非同侧连线。结论:传统针灸助产处方(合谷-三阴交)在促进分娩,增强药物流产方面具有积极作用;其电针参数可进一步筛选;作用机理可从促进子宫收缩,促进宫颈成熟的分子生物学角度深入探讨。
Objective: To review the studies on optimizing a classic acupuncture formula (LI4-SP6) for promoting labor, which have been done by our research group from 1996 to 2008, and to gain a perspective on the further research directions. Methods: Total 21 papers published by our research group were consulted so as to summarize and analyze those from the views of literature study, clinical study and mechanism study. Results : The literature studies showed that from the ancient times till now, LI4 and SP6 were the most frequently used acupoints for abortion or promoting labor in the clinic. The clinical studies suggested that electroacupunture (EA) at Hegu (LI4) and Sanyinjiao (SP6) could increase the uterine contraction, shorten the process of labor, and improve the effects of medicinal-induced abortion. Its mechanism might be related to inhibiting the disordered uterine contraction, enhancing the effective uterine contraction, and increasing the serum levels g of PGE2 and E2 respectively. The effects of needling sequence, needle retention duration, electric line connecting pattern of EA, and EA parameters on the therapeutic effects of acupuncture for promoting labor were summarized. Based on the results of repeated clinical and basic studies, a preliminary optimized formula was concluded as EA LI4 20 min first plus SP6 5mins later with 1 electric line (with 2 leads) on both LI4 and another line for both SP6. Conclusion: Classical acupuncture formula of LI4-SP6 has affirmative effects of promoting the uterine contraction and improving the therapeutic effect of medici- nal-induced abortion. Further studies on its EA parameters can be conducted in order to sifting the most effective combinations of EA parameters. The mechanism study can be performed by exploring the molecular biological mech- anism of uterine contraction and cervical ripening.
