摘要
目的探讨cine MRI在女性盆底功能障碍性疾病(pelvic floor disfunction,PFD)的诊断价值,为临床术前制定手术方案提供信息。方法2007年1至12月,以同济大学附属第十人民医院盆底功能障碍性疾病需手术治疗的住院患者20例为研究组,同期临床无盆底功能障碍性疾病症状和体征的患者15例为对照组。两组患者在平静状态和Valsalva动作时分别进行正中矢状位动态磁共振扫描,观察盆腔器官的位置和盆底形态。结果对照组膀胱颈、子宫颈、肛直肠连接位置静息时均位于耻尾线以上,盆腔用力时均有不同程度的下降,但平均下降幅度不大;研究组术前膀胱颈、子宫颈、肛直肠连接位置在盆腔用力时下降幅度明显。结论Cine MRI优于普通MRI扫描可用于全面评价盆腔器官脱垂和盆底形态;发现一些平时难以发现的轻微改变,对复杂盆底功能性疾病进行早期而全面的影像诊断,有助于临床手术方案的制定。
Objective To explore the diagnostic value of functional cine MRI in patients with pelvic floor dysfunction (PFD) , and to provide necessary information for designing the surgical procedures. Methods Twenty patients during January 2007 to December 2007 in Tenth People' s Hospital with findings of pelvic organ prolapse and pelvic floor dysfunction in clinical examination were assigned as the study group, and 15 asymptomatic female volunteers during the same time period without findings of pelvic floor dysfunction in clinical examination were assigned as the control group MRI at both Valsalva and relaxed situations, with the position. Results During the Valsalva maneuver, the All subjects underwent supine midsagittal dynamic changes of pelvic visceral position and pelvic floor positions of the bladder neck, cervix, and anorectal junction descended somewhat on straining, but not significant as compared with subjects in control group Meanwhile, these changes were very significant in subjects of the study group before surgery. Conclusion Functional cine MRI is a better and pelvic floor pattern which option than regular MRI, for it can accurately display pelvic visceral prolapse enable physicians to detect the trivial changes which might be neglected in regular MRI. Thus cine MRI plays an important role in imaging diagnosis and help design the appropriate surgical procedures.
