腰椎间盘突出自然演变的MRI观察 被引量:8

摘要 目的通过MRI跟踪观察腰椎间盘突出的自然演变。方法初患腰椎间盘突出症54例,分为膨隆型(19例),破裂型(20例)和游离型(15例)3组,定时复查MRI并测量矢状面下突出椎间盘造成的椎管狭窄程度和横截面下椎间盘突出占椎管的面积,观察不同病程时髓核突出的变化,平均观察时间18.6个月。结果随时间迁移,有28例(51.8%)突出椎间盘发生自行缩小变化,椎管狭窄率和椎间盘占位率呈不同程度的下降,其中膨隆型2例(10.5%),破裂型13例(65%),游离型13例(86.7%)。突出髓核在6个月内缩小最为明显,病程1年以后趋静止。结论椎间盘突出后突出椎间盘可发生自行吸收缩小,这种变化在突出椎间盘越大、纤维环损伤严重时较明显。
