Systematic review of the diagnostic value of the application of SELDI-TOF-MS technology in the analysis of serum protein fingerprint for distinguishing esophageal neoplasm

摘要 目的系统评价表面增强激光解析电离飞行时间质谱技术(SELDI-TOF-MS)检测血清蛋白组对食管癌的诊断效率。方法通过计算机检索Pubmed、Higwire等数据库和手工检索相关期刊全面检索当前使用SEL-DI-TOF-MS诊断食管癌的文献,并使用QUADAS工具对纳入的文献进行质量评价,采用MetaDisc1.4软件进行Meta分析,综合评价灵敏度、特异度、似然比、诊断优势比和综合接受者工作特征(SROC)曲线下面积(AUC)、Q*指数等指标。结果共检索到11个文献研究符合纳入标准,累积样本含量1350例。Meta分析结果显示,综合灵敏度为90.00%、特异度84.11%、阳性似然比5.09、阴性似然比0.14、诊断优势比39.17。SROC曲线分析,AUC=0.9160,Q*=0.8488。结论 SELDI-TOF-MS检测血清蛋白组对诊断食管癌具有较高的临床应用价值。 Objective To systematically assess the surface enhanced laser desorption/ionization time of flight mass speetrometry,SELDI-TOF-MS diagnostic tests for esophageal neoplasm and obtain the diagnostic value for esophageal neoplasm by SELDI TOF-MS. Methods Electronic searches were conducted in Pubmed, Higwire and so on. The related journals were also searched manually. Quality assessment of diagnostic accuracy studies(QUADAS) items were used to evaluate the quality of the studies. Meta-discl; 4 software was used to handle the data from the in- cluded studies. Indexes reflecting accuracy of SELDI-TOF-MS for diagnosing esophageal neoplasm such as sensitivity, specificity,likelihood ratio, diagnostic odds ratio, AUC(area under the curve) of SROC(summary receiver operat- ing characteristic) and Q index were pooled and evaluated. Results 11 studies that met the eligibility criteria included in 1350 subjects. Meta-analyses showed that sensitivity of SELDLTOF-MS for the diagnosis of esophageal neoplasm was 90.00 %, with specificity being 84.11%, PLR(positive likelihood ratio) being 5.09, NLR(negative likelihood ra- tio) being 0.14,and DOR(diagnostic odds ratio) being 39.17. The area under SROC curve was 0. 916 0 and Q index was 0. 848 8. Conclusion SELDI-TOF-MS can be regarded as an effective method for esophageal neoplasm diagnosis.
