手法与外固定器在桡骨远端不稳定骨折治疗中的应用

The Application of Manipulation and External Fixation in the Treatment of Unstable Fracture of Distal Radius

目的:总结手法与改良Ilizarov外固定器在桡骨远端不稳定骨折治疗中的应用经验。方法:2005年～2008年,采用手法复位,改良Ilizarov外固定器治疗30例桡骨远端不稳定骨折。其中,A3型12例,B2型3例,C1型8例,C2型4例,C3型3例。结果:全部病例均获得随访,采用Batra腕关节评分标准进行评分,其中优15例,良11例,可1例,差3例,平均86.27分。结论:手法与改良Ilizarov外固定器结合治疗桡骨远端不稳定骨折方法简单,疗效满意,是建立在一般桡骨远端骨折复位手法基础上的,借助器械和术后康复的的一套行之有效的方法。

Objective：To sum up the clinical experience of manipulation and improved Ilizarov fixator in treating unstable fractures of distal radius.Methods：From 2005 to 2008,30 patients with unstable fracture of distal radius were all treated with manual reduction and improved Ilizarov fixator.Among them,there were 12 in type A3,3 in B2,8 in C1,4 in C2 and 3 in C3.Results：All cases were successfully treated with the fixator,and all got following-survey.According to the criteria of Batra score of wrist,the excellent outcome was in 15 cases,good in 11,fair in 1 and poor in 3,with the mean score 86.27.Conclusion：The treatment for unstable fracture of distal radius with manipulation and improved Ilizarov fixator can obtain satisfied reduction and outcome.It is a simple and effective method including manipulation,external fixation and postoperative rehabilitation practices.
