体表心电图指导床边心脏临时起搏器植入的价值

The guiding of electrocardiogram to bedside temporary cardiac pacing by invading balloon-tipped floating catheter

目的探讨体表心电图对应用气囊漂浮电极床边植入心脏临时起搏器的指导价值。方法经不同静脉途径在体表心电图指导下应用气囊漂浮电极进行１５６例心脏临时起搏器的植入，５４例病例在随后植入永久性起搏器时进行了导管位置的Ｘ线观察，对比研究了起搏图形与导管位置，导管深度间的关系。结果体表心电图对床旁漂浮电极导管植入时具有较高的定位价值，适用于多数患者。心尖起搏组较流出道组起搏稳定，不易脱位。不同部位起搏阈值无显著差别。结论体表心电图结合已送入的导管深度对床旁漂浮电极导管进行快速准确心脏起搏具有重要指导意义。

Objective To evaluate the guiding value of electrocardiogram to bedside temperary cardiac pacing by invading balloon-tipped floating catheter. Methods 156 patients had received the therapy of temporary cardiac pacing via different venous by the guiding of electrocardiogram.54 patients of them were implanted the permanent pacemakers afterwards.During the procedure,the location of the catheters under fluoroscopy,and the relationship between the morphology of electrocardiogram and the tip position,depth of the catheters were studied.Results The electrocardiogram conduced to our decision of the catheter localization.The supervising of electrocardiogram during the procedure were suitable for majority and easy to manipulate.It also indicated that the pacing was stabler in the patients of apex pacing group than that of outflow tract group,and so is the fastness of catheters.There was no significant difference in the pacing threshold between the diverse pacing sites.Condusion The electrocardiogram is valuable in the guiding of the procedure of accurate and rapid bedside temporary pacing.
