摘要
目的对ＲａｍｓａｙＨｕｎｔ综合征（ＨＳ）临床和治疗进行分析，为了早期诊断和提高疗效。方法８４例ＨＳ病人平均年龄４８．３岁，男女比为２．２∶１，右侧发病多于左人侧（５２／３２）。先有疱疹的６３例，先有面瘫的２１例，自觉听力障碍的５７例，有眩晕的３４例，治疗以保守治疗为主，无效者行面神经减压术。结果半规管功能受损者２７例，中度以上听力受损者１１例。经过治疗面肌功能完全恢复的达５１．１９％，总有效率为９５．２３％。结论对ＨＳ要早期诊断，及时治疗，注意鉴别诊断，面瘫保守治疗无效考虑面神经减压术。先出现疱疹的要比先出现面瘫的预后好。
Objective To discuss the diagnosis and treatment of Ramsay Hunt syndrome(HS).Methods 84 cases of HS were analysed on its clinical characteristics, differentiation, treatment and prognosis.Results Average age of 84 cases of HS was 48.3 years; male versus female was 2.2 (1;the lesions were seen more often on the right than on the left (52/32).Of the 84 cases, 63 cases appeared auricle zoster at first, 21 cases appeared facial paralysis at first, 57 cases had dysaudia, and 34 cases had the symptom of vertigo. The diseases were treated mainly with such medicines as antivirus drugs, steroid, nutriments of nerve, etc. and facial nerve decompression was applied to those ineffective cases after conservative therapy.It was indicated that the etiology of HS is caricella zoster virus,which invades facial nerve,vestibular nerve and audiotory nerve,etc.27 cases had semicircular canal disorders,which were more often than neurosensory hearing loss (11 cases),51.19% of the patients had complete facial function recovery after treatment for several days to several weeks.The total rate of therapeutic effectiveness was up to 95.23%.Conclusion 1.The first early symptoms appearing usually indicates the heavily infected site,which usually lags behind in the recovery.2.Early diagnosis and differentiation are essential to HS.If the facial paralysis persists beyond 30 days without signs of recovery,decompression should be performed.3.Besides facial paralysis and auricle zoster,HS can invade vestibular nerve more often than auditory nerve.4.The patients contracting auricle zoster virus have better outcomings than the patients suffering facial paralysis.from other etiology.
