床突间隙的显微解剖及其在眼动脉段动脉瘤手术中的应用 被引量:4

Microsurgical Anatomic Study of Clinoid Space and Its Clinical Application in Microsurgery of Carotid Ophthalmic Aneurysms

摘要 目的:为临床上磨除前床突形成床突间隙提供显微解剖学数据,研究磨除前床突在眼动脉瘤夹闭中的应用价值。方法:在手术显微镜下对10例(20侧)成人尸头的前床突及床突间隙进行显微解剖观测,并在临床上采用磨除前床突法,夹闭眼动脉段动脉瘤12例。结果:床突间隙为磨除前床突后形成的椎形空间,可以提供0.8-1.0cm3的显露空间,12例眼动脉段动脉瘤均利用该间隙成功夹闭。结论:床突间隙增加了手术空间,通过该间隙可处理颈内动脉眼动脉段及床突段动脉瘤。 Objective: To provide detailed anatomic data of clinoid space and its clinical application in clipping ophthalmic aneurysms.Methods: The anterior clinoid process(ACP) and clinoid space(CS) of 10 adult cadaver head specimens(20 sides) were observed and measured under operating microscope.Twelve cases of carotid ophthalmic aneurysms were cured with the direct microsurgical clipping after the ACP was stripped.Results: CS was a wedge space,and after the ACP was removed,it could provide about 0.8-1.0 cm3 additional operating space.Twelve cases of carotid ophthalmic aneurysms were clipped after ACP was removed.Conclusion: CS could provide a larger operation space for clipping the carotid ophthalmic aneurysms.
