多节变形固定板的生物力学测试及临床应用研究

摘要 [目的]探讨多节变形固定板的生物力学特点及临床应用价值.[方法]①取6套多节变形固定板和6套柳木夹板, 随机分为6组.在不同作用力下,测出两种夹板受力后产生的纵向位移及撤销外力后的剩余位移, 绘制相应曲线图进行分析.②80例桡骨远端伸直型骨折患者被随机分两组(多节变形固定板组和柳木夹板组各40例).治疗4周、8周后观察患者的主、客观及X线片表现, 依据Jakim评分标准进行评分,了解骨折愈合情况、复位固定效果、患肢功能恢复状况.[结果]多节变形固定板强度及弹性明显优于柳木夹板(P〈0.05);经多节变形固定板治疗后的骨折愈合情况、复位固定效果、患肢功能恢复均优于传统柳木夹板(P〈0.05).[结论]多节变形固定板是一种理想的外固定材料,力学性能稳定, 弹性和强度均能满足临床要求,临床疗效满意. [Objective] To explore the blomechanics characteristics and clinical value of multi-segment joint amorphous fixation brace. [Methods] Six series of multi-segment joint amorphous fixation brace and six series of willow splints were divided into six groups randomly. Under different working forces, length wise offset and residue offset were measured, and curve diagram was drawn to analyze. Eighty cases with Colles fracture were randomly divided into multi-segment amorphous fixation brace group and willow splint group with 40 cases for each group. The objective and subjective manifestation and X ray were observed at 4 weeks and 8 weeks after treatment. According to Jakim criteria, healing time, fixation effect and function recovery were evalua- ted. [Results]Intensity, elasticity, healing time, fixation effect and function recovery in multi-segment joint amorphous fixation brace group were better than willow splint group. [Conclusion] Multi-segment amorphous fixation brace has stable mechanical property and satisfactory effect, and its elasticity and strength can accord with clinical demand, so it is an ideal external fixation system.
