经肛单吻合器法在低位直肠癌保肛手术中的应用

TRANSANAL SINGLE-STAPLING TECHNIQUE FOR ANUS-PRESERVING SURGERY IN LOW RECTAL CANCER

目的探讨经肛单吻合器法在低位直肠癌切除保留肛门括约肌功能手术(保肛手术)中的应用效果。方法低位直肠癌病人61例,在严格遵行TME的前提下,采用经肛单吻合器技术行保肛手术。结果 61例全部完成保肛手术,2例术后2周内发生吻合口漏,3例发生吻合口狭窄,无手术死亡。随访6个月～4年,平均3.4年,局部复发3例。结论经肛单吻合器技术用于低位直肠癌前切除保肛手术效果较好,值得临床推广应用。

Objective To explore the use of single-stapling （SS） technique in anal-sphincter-preserving surgery for lowset rectal cancer. Methods Using SS technique, an anus-preserving surgery was performed in 61 patients with low-set rectal cancer acted on TME premise. Results The surgery was completed in all the patients. Within two weeks postoperatively, storeal leak occurred in two cases, and stoma stricture in three, with no operative death. The mean （range） following-up of 41 （6-- 48） months showed local recurrence in three cases. Conclusion Single-stapling technique applied in sphincter-preserving surgery for low rectal cancer is effective and worth clinical generalization.
