摘要
目的观察比较手足口病(HFMD)患儿和健康儿童心肌酶谱、肾功能和尿视黄醇结合蛋白的水平,了解手足口病对患儿心肌损伤和肾脏损害,为临床诊断和治疗提供参考依据。方法对40例手足口病患儿(观察组)分别进行血清心肌酶谱、血肌酐、尿素氮和尿视黄醇结合蛋白检测;对照组为30例健康体检儿童。结果观察组患儿心肌酶谱和肾功能均有不同程度改变,乳酸脱氢酶增高20例占50.0%,谷草转氨酶增高7例占17.5%,磷酸肌酸激酶增高2例占5.0%,磷酸肌酸激酶同工酶增高22例占55.0%,尿素氮增高1例占2.5%,尿视黄醇结合蛋白增高23例占57.5%,观察组患儿心肌酶谱和肾功能与对照组比较,差异有统计学意义(P<0.01)。结论手足口病患儿心肌酶谱、血尿素氮和尿视黄醇结合蛋白的结果明显高于对照组;手足口病患儿在心肌损伤的同时还存在肾小管的损害;心肌酶谱、肾功能和尿视黄醇结合蛋白的检测有利于早期脏器损害的评估,为临床诊断和治疗提供参考。
OBJECTIVE To compare the serum myocardial enzyme,serum renal function and serum urinary retinol binding protein of the HFMD children and normal controls,to illustrate myocardial and renal damages in children with HFMD,thus provide bases for clinical diagnosis and treatment.METHODS Totally 40 HFMD children and 30 normal children were set as the study and the control group,the serum myocardial enzyme,serum creatinine,blood urea nitrogen and serum urinary retinol binding protein were tested.RESULTS The serum myocardial enzyme and serum renal function in the study group were different from that in the control group.In HFMD group,the increase of LDH was in 20 cases(50.0%),increase of AST was in 7 cases(17.5%),increase of CK was in 2 cases(5.0%),increase of CKMB was in 22 cases(55.0%),increase of BUN was in 1 case(2.5%),increase of RBP was in 23 cases(57.5%) and no change in CR.The differences were statistical significant between HFMD group and control group according to LDH,AST,CK,CKMB,BUN,RBP(P0.01).CONCLUSION The myocardial enzyme,serum urea nitrogen and urinary retinol binding protein of children with HFMD demonstrate significantly higher than that in control group.The HFMD children who have the myocardial damages also have the renal tubular damages,the detection of serum myocardial enzyme,serum renal function and serum urinary retinol binding protein will benefit the estimation of early renal damages and provide laboratory support for the clinical diagnosis and treatment.
