摘要
目的: 探讨食管癌切除两种不同重建方法的优劣。方法: 通过对230 例食管中段癌切除后,在胸顶部以上( 超胸顶组) 用吻合器行食管胃吻合160 例、颈部手工吻合( 颈部组)70 例重建消化道的对比研究。结果: 两组在食管切除长度、吻合口瘘、食管上切缘癌残留及总体死亡率方面均无显著差异( P> 0 .05) ,而在手术时间、减轻手术损伤、减少吻合口狭窄的发生率方面,超胸顶组明显优于颈部组( P< 0 .05) 。结论:
Objective: Discussion of esophageal carcinoma of midpiece resecting two regions and reconstruction methods.Methods: A comparisive study on reconstruction of the digestive tract was carried out in 230 cases of resection of esophageal carcinoma of midpiece,160 cases of esophagogastrostomy at the thoracic opex with instrument of anastomosis(group Ⅰ);70 cases of anastomosis at neck by hand (group Ⅱ).Results: No difference in esophageal resected lenth,anastomotic fistula,carcinoma residual rate between these 2 groups ( P >0.05),and group Ⅰ was superior to group Ⅱ in shortening operation time,abatement operative injury,reducing incidence of anastomotic stenosis ( P <0.05).Conclusion: The method of group Ⅰ was ideal and effective to reconstruct esopagus after resection of esophageal carcionoma of midpiece.
