老年糖尿病患者86例低血糖的研究

目的研究86例老年糖尿病患者低血糖的原因及健康教育的干预效果。方法将我院2009年住院的86例老年糖尿病低血糖患者作为研究对象,分析低血糖的临床表现、危害、防治、原因及干预效果。结果老年糖尿病患者低血糖的原因是不合理使用降糖药物(38.4%)、不合理使用胰岛素(24.4%)、摄入食物不足(16.3%)、发热感染(11.6%)、运动过量(5.8%)、空腹饮酒(3.5%)。经过积极的健康教育和临床治疗,治愈率为100%,其中24.2%患者出现夜间低血糖症状。健康教育后患者的血糖(空腹血糖、餐后2 h血糖、糖化血红蛋白)均在正常范围,与干预前相比差异有统计学意义,P<0.001;自我血糖控制良好(合理饮食、血糖监测、合理运动)的患者数显著多于干预前,P<0.001。结论老年糖尿病患者低血糖比例较高,加强对患者低血糖预防和治疗的健康教育,对预防低血糖的发生、减轻低血糖的危害有重要意义。

Objective To study the reasons and results of health educations for low blood sugar of 86 cases of elderly patients with diabetes.Methods Take 86 cases of elderly patients with diabetes and low blood sugar who hospitalized in 2009 as research subjects.Analyze the clinical manifestations of hypoglycemia,harm,prevention,causes and intervention.Results The reasons of elderly patients with diabetes and low blood sugar are unreasonably using hypoglycemic agents（38.4%）,unreasonably using insulin（24.4%）,inadequate food intake（16.3%）,fever,infection（11.6%）,excessive movement（5.8%）,fasting alcohol（3.5%）.After a positive health education and clinical treatment,the cure rate was 100%,of which 24.2% patients with nocturnal hypoglycemia.After health education,patients＇ blood glucose（fasting glucose,postprandial 2 h blood glucose,glycosylated hemoglobin）were within normal range,compared with before intervention significantly,P〈0.001;Self blood glucose control（proper diet,blood glucose monitoring,reasonable exercise） well,were significantly to before intervention,P〈0.001.Conclusion There＇s high proportion of low blood sugar for elderly patients with diabetes.Strengthen prevention and treatment and do health education of hypoglycemia is important for preventing the occurrence and reducing the dangers of low blood sugar.
