利多卡因表面麻醉手法小切口白内障人工晶体植入疗效观察

Observation on the treatment effect of applying mieroineision Intraoeular lens implantation on cataracts under superficial lidocaine anesthesia

目的探讨2％利多卡因表面黏膜麻醉非超声乳化人工晶体植入手术效果。方法在76例（87眼）白内障复明手术中，采用一字型或反眉形巩膜隧道小切口，水分离，娩出晶状体核，注吸皮质，囊袋内植入人工晶状体，切口不予缝合。结果92％患者仅靠表面麻醉即可完成手术，术后第1天裸眼视力≥0．5者占87．3％，其中≥1．0者占27．5％；术后1周视力≥1．0者占51．9％，术后1月视力≥1．0者占63．3％。术后3～6个月角膜散光为（0．75±0．79）D，与术前比较无差异（P〉0．05）。结论利多卡因表面麻醉小切口非超乳人工晶体植入易操作、成本低、质量高，值得推广。

Objective To investigate the treatment effect of applying microincision Intraocular lens implantation on cataracts under superficial lidoeaine anesthesia. Methods Used reverse camber sclera tunnel microincision incision, water separation, childbirth lens nuclear, note suck cortex, bursa bag inside implanted artificial lens to treat all these 76 cases （ 87 eyes ） that would have cataract surgery operation without suturing the incision. Besults 92 % patients could finish the operation just under surface anaesthesia. On the first postoperative day, 87.5% cases＇ visual ability ≥ 0.5, and 27.5% cases visual ability ≥ 1.0. After a week of the operation 51.9% cases＇ visual acuity ≥1.0. After a month of the operation 63.3% cases＇ visual acuity ≥1.0. After 3 - 6 months, the corneal astigmatism is （ 0.75 ± 0.79 ） D. The visual ability of all patients after the operation had no difference from all the patients＇ preoperative visual ability （ P 〉 0.05 ）.Conclusions 2% lidocaine surface mucosa was under anesthetic technique microincision incision nonphacoemulsification phacoemulsification and intraocular lenses implanted operation, simple, safe, the pain is small and the visual acuity better, has obtained the low cost, high quality effect, is worthy to be popularized.
