摘要
目的探讨尿微量清蛋白(mAIb)、α1-微球蛋白(α1-MG)、β2-微球蛋白(β2-MG)、免疫球蛋白G(IgG)对糖尿病肾病(DN)的早期诊断价值。方法按尿清蛋白排泄率(UAER)及糖化血红蛋白(HbA1c)将患者分组,采用放射免疫法(RIA)检测24 h尿mAlb、α1-MG、β2-MGI、gG的含量,分别进行统计学分析。结果 UAER正常组尿α1-MG、β2-MG水平均显著高于对照组,UAER增高组尿4种蛋白均显著高于对照组。血糖控制良好组的2型糖尿病患者,其尿中的mAlb、α1-MG、β2-MGI、gG水平显著低于血糖控制不良者组。结论尿多种蛋白联合检测对糖尿病肾脏病变的早期发现和定位有较大的诊断价值。
Objective To explore the value of combined measurement of urinary mAlb,α1-MG,β2-MG,IgG in diagnosing diabetic nephropathy at early stage.Methods The levels of 24 h urine mAlb,α1-MG,β2-MG,IgG were measured with RIA.According to the albumin excretion rate(UAER) and the glycosylated hemoglobin(HbA1c),the patients were divided into two groups.Statistical analysis were accomplished based on the contents of 24 h urine mAlb,α1-MG,β2-MG and IgG.Results The levels of urine α1-MG,β2-MG in normo-UAER group patients were much higher than those in normal control.Four kinds of urinary protein in hypsi-UAER group increased significantly.The levels of urine mAlb,α1-MG,β2-MG,IgG in plasma glucose well-controlled group patients were much lower than those in plasma glucose poorly-controlled group.Conclusion The combined measurement of several urinary proteins is important for the early diagnosis and determination of diabetic kidney disease.
