摘要
目的 探讨超敏C反应蛋白(high sensitive C-reactive protein,Hs-CRP)对肺部感染性疾病病情、预后判断的价值和意义.方法 回顾性分析2009年1月~2009年12月我院呼吸科133名单纯性肺炎患者、68名肺炎合并轻度脓毒症患者和22名肺炎合并重度脓毒血症患者临床资料[1],血清hs-CRP采用乳胶增强免疫比浊法检测[2].结果 单纯肺炎组、合并轻度脓毒症组和合并重度脓毒症组抗生素治疗前超敏CRP分别为36.7±30.8mg/L、88.3±22.8mg/L、144.6±55.8mg/L.治疗一周后三组Hs-CRP水平分别为17.7±11.8mg/L、50.3±13.8mg/L、99.6±25.8mg/L,治疗前三组间数据存在统计学差异(P<0.05).与治疗前相比,治疗后三组Hs-CRP水平均存在统计学差异.而治疗前轻度脓毒症组与重度脓毒症组中性粒细胞检测无明显差异(10.7±3.8×10^9/L vs.13.1±6.8×10^9/L,P>0.05).抗生素治疗一周后,重度脓毒血症组白细胞(14.8±6.8×10^9/L vs.13.1±6.8×10^9/L)和中性粒细胞计数(13.1±6.8×10^9/L vs.12.1±4.4×10^9/L)较治疗前均无显著差异(P>0.05).结论 hs-CRP是反映肺部感染性疾病的严重程度及预后判断有价值的诊断指标.
Objective to explore the value of high sensitive C- reactive protein(Hs- CRP)in severity evaluation, therapeutic effect and prognosis of pulmonary infectious diseases. Methods clinic data of 133 mild pneumonia patients, 68 pneumonia patients with sepsis and 22 pneumonia patients with severe sepsis in department of respiratory diseases were analyses retrospectively. Serum Hs - CRP was determined b, Latex enhanced immunoturbidimetric assay. Results Hs - GRP in mild pneumonia group, pneumonia with sepsis group and pneumonia with severe sepsis group were 36.7 ± 30.8mg/L,88.3 ± 22.8 mg/L, 144.6 ± 55.8 mg/L, respectively, before antibiotie therapy, After antibiotic therapy for one week, Hs - CRP were 17.7 ± 11.8 rag/L,50.3 ± 13.8 rag/L,99.6 ± 25.8 rag/L, respectively. There had a significant dig ferenee ( P 〈 0.05) among three groups before therapy, and Hs - CRP showed a significant difference ( P 〈 0.05 ) after antibiotic therapy in each group. While neutrophil count between pneumonia with sepsis group and pneumonia with severe sepsis group showed no significant differenee(P 〉 0.05)before antibiotic therapy, White blood cell count and neutrophil count in pneumonia with severe sepsis group before and after antibiotic therapy showed no significant differenee(P 〉 0.05). Conclusion Hs - CRP is an effective and sensitive indicator to evaluate the therapie sensitivity and prognosis in pulmonary infectious diseases.
