社区全科医师知识技能现状与培训需求研究

Status of Community General Practitioners' Knowledge and Skills and Their Demand for Training

目的了解社区全科医师的全科服务相关知识和技能掌握情况,为制定合理的全科医师培养计划提供依据。方法对上海市徐汇区全部12家社区卫生服务中心所有的注册全科医师进行自填式问卷调查。结果知识技能标化总分平均值为(79.20±10.90)分,按类别标化分从高到低依次为:健康教育、预防、保健、康复、计划生育和医疗。全科医学培训中存在的前三位主要问题依次为:培训与工作时间冲突(65.07%)、缺少临床实践(54.45%)和培训深度不够(47.95%);社区全科医师最需要补充的前三位知识或技能依次为临床实践技能(69.97%)、临床理论知识(42.32%)和社区工作经验(41.64%)。结论完善社区全科医师培训系统,加大针对临床医疗等薄弱方面的知识和实践培训,鼓励参加多形式、多领域的培训,提高社区全科医师的综合素养,推动社区卫生服务发展。

Objective To study and analyze the status of community general practitioners＇ knowledge and skills related to the general service so as to provide the basis for formulating the reasonable training plan for the practitioners. Methods The questionnaire survey was conducted among all registered general practitioners working in all the 12 community health service centers of Xuhui district,Shanghai. Results The total average standardized score of expertise capability was（79.20±10.90）.The standardized scores of different fields rank from high to low were in health education,prevention,health care,rehabilitation,family planning and medical service.The first three main problems reported in the general practice were time conflict between training and working（65.07%）,lack of clinic practice（54.45%） and limited content of training（47.95%）.And the first three identified as major indications for general practitioners were clinical practice skill（69.97%）,clinic theoretical knowledge（42.32%） and community practice experience（41.64%）. Conclusion It is essential to develop the training system for general practitioners,to enhance the training on clinical medical knowledge and practice that need to be improved,and to encourage general practitioners to participate in training programs in multiple fields and types so as to raise their comprehensive qualities and promote the development of community health services.
